I read the article "Managing Incomplete Tooth Fractures" by Dr. J. Edward Ailor Jr. (August JADA). This is a very fine, comprehensive, detailed article on causes and treatment of vertical tooth fractures, both complete and incomplete.
I notice on page 1171 that dentists are advised to be sure their handpiece is vibration-free and that burs and diamonds run true. However, there is no reference to the use of water spray, especially in preparing endodontically treated teeth that have a history of potential vertical fractures.
Because there is no need for pulp protection, there is perhaps a tendency not to use water spray. However, the heat generated by dry cutting can enhance further checking and cracking of tooth structure.
The dentist should be advised to use extensive water spray when preparing nonvital teeth for future crown preparations. In addition, vital teeth with a history of potential vertical faults also require maximum water spray to keep the surface temperature as ambient as possible.
Again, I would like to congratulate Dr. Ailor on a very important, informative contribution to dental literature.
